Our Social Prescribing service Connect for Health has provided invaluable support to people of Leeds South & East for almost 4 years. From Friday 30th September the Connect for Health service will end as it transitions, along with PEP and Connect Well, to start a new and exciting city -wide Social Prescribing service called Linking Leeds. Connect for Health are therefore closed to new referrals.
Existing clients of the service can still contact their Wellbeing Coordinator on their mobile number but we are asking anyone enquiring about how to refer to contact Linking Leeds on 0113 336 7612 after Monday 9th September.
We would like to thank you for your support over the last four years. We are very excited to be able to offer a city wide service from Monday 2nd September.

Social Prescribing services in Leeds will support you to access local community groups and activities to help you make a positive change and improve your mental health and wellbeing. You will be supported either in a group or in a one-to-one setting to help you to develop the skills, knowledge and confidence to self-manage your condition.

Connect For Health
Connect for Health is a service currently available to people living in South Leeds and East Leeds. Connect For Health assesses each individual’s needs and takes into account the challenges you are facing, before providing you access to a personalised selection of local groups and activities which could help you achieve positive mental health. Patient Empowerment Project
PEP (Patient Empowerment Project) is a service currently available for people living in West Leeds. With a referral from your West Leeds based GP, you will meet with PEP, who will assess your needs, and provide you with access to a selection of local community groups and activities, with the aim of helping you to achieve positive mental health.
